29 CFR 1910.22(d)(1): Walking-working surfaces were not inspected, regularly and as necessary, and maintained in a safe condition: a) Bindery Department - On or about July 20, 2017, the employer did not ensure that the floor was maintained in a safe condition. The damaged floor exposed employees to trip and struck-by hazards when walking and moving equipment with pallets of product in this area.

29 CFR 1910.303(b)(2): Listed or labeled equipment were not used or installed in accordance with any instructions included in the listing or labeling: a) Bindery Department - On or about July 20, 2017, the employer did not ensure that the metal four-plex receptacle box was installed in accordance with its listing or labeling. The receptacle box was installed on the end of a flexible cord and used to provide electricity to a strap machine.
