29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. On or about 7/20/17 located at 4335 Atwood Dr., Cumming, GA: Employees were exposed to fall hazards up to 21- feet from the lower level, while performing framing work from the make shift scaffolds made from 2 X 8 lumber. The employer failed to install guard rails.

On or about 7/20/17 located at 4335 Atwood Dr., Cumming, GA: Employee (s) were installing rafters and plywood on to the house that was under construction while working at the edge, the employer did not ensure that fall protection was used by the employees while exposed to fall hazards of approximately 14 - 25- foot on the 8/12 pitch roof.
