29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form or scrap lumber with protruding nails was not kept clear from work areas:    (a)  On or about July 19, 2017, and at times prior, employees were exposed to lacerations, impalement hazards, slips, trips and falls due to inadequate housekeeping, including, but not limited to protruding nails from scrap lumber throughout the   third floor of building 13. (subcontractor) protected their employees from fall hazards of 30 feet and greater while engaged in residential construction activities. 29 CFR 1926.502(b)(2): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structure members were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches (53 cm) high.    (a)   On or about July 19, 2017, and at times prior, employees were exposed to fall hazards in excess of six feet in that a mid-rail was not installed along the guardrail that was installed adjacent to the open sided floor. 29 CFR 1926.1052(c)(12): Unprotected sides and edges of stairway landings were not provided with guardrail systems that meet the criteria contained in Subpart M of 29 CFR 1926:  (a) On or about July 19, 2017, and at times prior, employees were exposed to fall hazards in excess of six feet in the stairway landing in between the second and third floor where the guardrail was missing a mid-rail.
