29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: a) On or about July 12, 2017; east-side (front) of the project; employees were working in an aerial lift (Genie S-60X series) without fall protection exposing them to an estimated 30-35 foot to ground level. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. a) On or about July 12, 2017; east-side (front) of the project; employees working in an aerial lift (Genie S-60X series) were not trained on fall hazards or the use of fall protection.
