Employees were not certified and evaluated according to the specifications in this standard.

On or about July 19, 2017 and there prior, employees operating the rough terrain forklift at the residential construction site did not complete formal instruction training on Powered Industrial Trucks. Employees did not have the requisite knowledge to operate the equipment and lift materials in a safe manner.
