29 CFR 1928.57(a)(6): Operating instructions. At the time of initial assignment and at least annually thereafter, the employer shall instruct every employee in the safe operation and servicing of all covered equipment with which he is or will be involved, including at least the following safe operating practices: On or about July 3, 2017 at the N15927 County Road G, Nekoosa jobsite; the employer did not instruct in safe operations for obtaining haylage samples by allowing samples to be taken in the blind spot of the equipment. This exposed employees to struck-by hazards.

29 CFR 1928.57(a)(7): Methods of guarding. Except as otherwise provided in this subpart, each employer shall protect employees from coming into contact with hazards created by moving machinery parts as follows: On or about July 3, 2017 at the N15927 County Road G, Nekoosa jobsite; the wheels located by the infeed table of the Miller Ag-Bag X1114 Professional (S/N: 42 05155) were not guarded exposing employees to struck by and run over hazards.
