29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards.  Employees engaged in residential roofing activities were not provided training by the employer to recognize the hazards of falling and the procedures to be followed to minimize these hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   Employees engaged in residential roofing activities using a 24 foot portable Werner aluminum extension ladder, model D1824-2EQ were exposed to fall hazards of approximately 11 feet from the eave of the structure to the ground below in that the ladder used to access the roof was not extended at least 3 feet above the eave and was not secured to the structure. The side rails of the ladder were extended approximately 18 inches above the landing surface of the roof.
