29 CFR 1926.21(b)(2): The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury.  At the residential construction site located at 502 Lexington Ave, Independence, MO:  Employees were exposed to fall hazards. The employer had not trained employees in the recognition, evaluation, and control of fall hazards.  Two employees were exposed to fall hazards while working from a roof and had not been trained in the regulations applicable to their work.

29 CFR 1926.1053(b)(22): Employees  using ladders were carrying objects or loads that could cause the employee to lose balance and fall:  At a residential construction site located at 502 W Lexington Ave, Independence MO:  Employees were exposed to fall hazards.  Roofing employees were observed carrying heavy bundles of roofing materials up an extension ladder that substantially increased the potential for loss of balance resulting in a fall.

29 CFR 1926.102(a)(1): The employer should ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  At the residential construction site located at 502 W Lexington Ave., Independence, MO: Employees were exposed to struck-by hazards. Roofers were observed working without eye protection while using a Hitachi pneumatic nail gun without the use of eye protection.  RLL GENERAL CONTRACTORS,LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(11) which was contained in OSHA inspection number 1194005, citation number 1, item number 4 and was affirmed as a final order on 03/15/2017, with respect to a workplace located at 1709 Tudor Lane, Liberty, MO 64068.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  At the residential construction site located at 520 W Lexington Ave. in Independence, MO:  Employees were exposed to fall hazards. Roofers were removing and installing roofing materials without the use of fall protection. Roofers were exposed to fall distances of approximately 9 feet from the eve of the roof to the ground below.  RLL GENERAL CONTRACTORS,LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13) which was contained in OSHA inspection number 1236767, citation number 1, item number 1 and was affirmed as a final order on 07/03/2017, with respect to a workplace located at 3800 NE 79th Terrace, Kansas City, MO 64119.  RLL GENERAL CONTRACTORS,LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(11) which was contained in OSHA inspection number 1194005, citation number 1, item number 4 and was affirmed as a final order on 03/15/2017, with respect to a workplace located at 1709 Tudor Lane, Liberty, MO 64068.

Roofing employees were observed using portable extension ladders to gain access to the roof.  The side rails of the ladder did not extend three feet above the upper landing surface of the roof.
