29 CFR 1910.28(b)(1)(i): Each employee on a walking-working surface with an unprotected side or edge 4 feet or more above a lower level was not protected from falling by guardrail systems, safety net systems, or personal fall protection system. (a) Backup Roll Pit on 9 Tandem Mill: On July 13, 2017, an employee was performing maintenance activities, accessing an open pit via support beams 8 to 10 feet from lower level without the use of a fall protection system.
