On or about July 22, 2017, an employee was exposed to struck-by and caught-in hazards when a forklift was driven up to the employee occupying an area between the loaded forklift and a stationary metal A-Frame glass storage rack.  The forklift load of approximately nineteen 5'x10' glass sheets became unbalanced and collapsed onto the employee causing fatal injuries.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
