29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: (a) Top of framed roof - On or about July 20, 2017 two employees were working from the framed roof rafters with no fall protection to keep them from falling 12 feet.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: (a) Rear of house - On or about July 20, 2017 employee working on top of framed wall attaching roof rafters gained access to top using a 16 feet aluminum extension ladder that did not extend 3 feet past roofs edge for easy and safe access to the roof.
