29 CFR 1910.22(d)(1): The employer did not ensure that walking-working surfaces were maintained in a safe condition:  Vehicle Inspection Area (Vehicle Lift): The walking-working surface surrounding the vehicle lift had an abrupt change/void in height of approximately 7 inches in the surface areas between the curb and the lift pad exposing employees to trips and falls.

29 CFR 1910.151(b): First aid supplies approved by a consulting physician were not readily available:  Operations Office: The first aid cabinet had minimal supplies exposing employees to further injury if injured due to the lack of first aid supplies in case of an emergency.  The cabinet was lacking tweezers, eye drops, pain relief medicines, burn spray, an assortment of gauze, an assortment of nitrile gloves, and antiseptic wipes.

29 CFR 1910.178(a)(6): The employer did not ensure that all nameplates or markings were not maintained in a legible condition:  Vehicle Inspection Area: The Clark sit down forklift truck (CGP 25) had a name plate that was not legible and capacities were unidentifiable.  Employees may lift loads greater than the allowance causing the forklift to tip or become unstable.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  West Port of Entry:  Employees use narcotic testing kits which contain hazardous chemicals.  Employees did not know what the chemicals were in the testing kits and the associated hazards.  Employees did not know what to do for first aid after skin contact or eye contact exposing employees to skin irritation and eye irritation.

29 CFR 1904.29(b)(1): A log of all recordable work-related injuries and illness (OSHA Form 300 or equivalent) was not completed in the detail required by the regulation.  Calexico Port of Entry: The cases on the 2017 OSHA Form 300 were not completed in detail.  The information described on the form did not include what object/substance directly injured or made the person ill.    The U.S. Customs and Border Protectoin was previously cited for a violation of this occupational safety and health standard or its equivalent standard [29 CFR 1904.29(b)(1)], which was contained in OSHA Inspection number 1181665, citation number 2 item number 1 and was affirmed as a final order on January 11, 2017, with respect to a workplace located at 200 East First Street, Calexico, CA, 92231.
