29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:  On or about July 19, 2017:  a) An employee utilizing a pneumatic nail gun to install shingles on a new residential structure was exposed to eye injuries when the employer failed to ensure that the employee was using protective eye wear while preforming job related tasks.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.502(d)(17): The attachment point of the body belt was not located in the center of the wearer's back. The attachment point of the body narness was not located in the center of the wearer's back near shoulder level, or above the wearer's head.  On or about July 19, 2017:  a) Employee was exposed to injury when the employer failed to ensure that the fall protection measure he was wearing was appropriate for the job tasks required. The employee was wearing a rock climbing type harness that has an attachment point in the front of the wearers body.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about July 19, 2017:  a) the employer failed to properly train his employees in the recognition and the proper corrective steps when they were exposed to fall hazards. Employee was utilizing a rock climber type harness with the attachment point was located in the front of the wearers body.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:(a) (LOCATION) (IDENTIFY SPECIFIC OPERATION(S) AND/OR CONDITIONS)  On or about July 19, 2017:  a) Employee was exposed to a fall hazard of approximately 30 feet when the ladder he was utilizing to gain access to the working area did not extend past the landing at least three feet.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
