29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects: a) Exterior South Side of Building: An employee engaged in trench work inside a trench with a depth greater than five feet, was not wearing a protective helmet to protect against cement being poured in to the trench. The employer did not ensure that employees were protected with protective helmets, thus exposing employees to a head injury, on or about, 7/21/17.
