On or about July 24, 2017, on the south side of the plywood shed, the storage platform dock did not have standard railings to keep employees or equipment from falling over the sides, exposing employees to an 11 feet 6 inch fall hazard.

On or about July 24, 2017, the employer did not ensure employees operating a two Cobra 8,000 lbs. capacity forklift and a Mitsubishi 10,000 lbs. capacity forklift daily had successfully completed training and were evaluated to operate the forklift, exposing employees to struck-by and crushing hazards.
