29 CFR 1926.502(f)(1)(i): Warning line was not erected at least 6 feet (1.8m) from the roof edge: 10 Centerville Rd., Holmdel, NJ: On or about July 24, 2017, Garden State Roofing and Sheet Metal Inc. employees were conduct roofing work on the large flat roof of a warehouse facility under renovation. A warning line had been erected as fall protection. The warning line was 4 feet from the roofs edge, exposing employees to potential fall hazards. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.502(f)(1)(iii): Points of access, material handling areas, storage areas, and/or hoisting areas were not connected to the work area by an access path formed by two warning lines: a) Ladder access area, 10 Centerville Rd., Holmdel, NJ: On or about July 24, 2017, Garden State Roofing and Sheet Metal Inc. employees were conduct roofing work on the large flat roof of a warehouse facility under renovation. A warning line had been erected as fall protection. There was no delineated warning line path to the access point and there was not a rope, wire, chain, or other barricade at the point of access to ensure that workers cannot walk directly into the hazardous roof edge area. This created a potential fall hazard to the ground below. b) Material handling / hoist area, 10 Centerville Rd., Holmdel, NJ: On or about July 24, 2017, Garden State Roofing and Sheet Metal Inc. employees were conduct roofing work on the large flat roof of a warehouse facility under renovation. A warning line had been erected as fall protection. There was no delineated warning line path to the hoist area. There was not a rope, wire, chain, or other barricade to ensure that workers could not walk directly into the hazardous roof edge area. This created a potential fall hazard to the ground below. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

The warning line was not properly supported and in fact sagged to the roof's surface at several points, exposing employees to a potential fall hazard from the roof to the ground below.
