29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers. On July 25, 2017, located on the south side side of a commercial roof structure, employees were installing roofing insulation at the leading edge of a low pitch roof as there were no frequent and regular inspections being performed by a competent person to ensure the job-site, materials and equipment such as a fall protection systems and/or devices were being maintained and utilized in a safe condition. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1926.501(b)(10): "Roofing work on Low-slope roofs." Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width (see Appendix A to subpart M of this part), the use of a safety monitoring system alone [i.e. without the warning line system] is permitted. Located on the south side of a commercial structure, employees were observed installing roofing insulation on the edge of a low slope pitch roof structure while not utilizing an adequate fall protection system. Potential Fall Heights were measured to be 13'-2" with a roof pitch of 2/12.
