29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees.  On or about 26 July 2017, and at times prior thereto, three plumbers were working in an excavation that was over four feet in depth without a means of safe egress, exposing the workers to a fall hazard of approximately 4 feet 9 inches.

29 CFR 1926.651(i)(3): Sidewalks, pavements, or appurtenant structures had been undermined and a support system or another method was not provided to protect employees from the possible collapse of such structures.  On or about 26 July 2017, and at other times prior thereto, three plumbers were working within a small four foot wide tunnel that ran approximately 10 to 15 feet underneath a sidewalk and foundation without a support system, exposing the workers to the hazard of being caught within the tunnel during a collapse of such structures.

29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary.  On or about 26 July 2017, and at times prior thereto, three plumbers were working within an excavation in which the spoil pile was allowed to accumulate at the edge, exposing the workers to the hazard of being struck by falling rocks, asphalt, dirt, tools and materials.
