29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, and rotating parts: a) Worksite: The employer did not ensure that Bonko Controls - Machine Technologies & Control, Inc. Straightener #3, Asset #2593, Equipment #ST03, Serial #501 187 was equipped with adequate rotating part guarding to prevent employees from having any part of their body enter into the danger zone during the operating process. The machine was able to be operated with rotating parts exposed; on or about 7/24/17. b) Worksite: The employer did not ensure that Bonko Controls - Machine Technologies & Control, Inc. Straightener #2, Asset #2591, Equipment #ST01, Serial #501 185 was equipped with adequate rotating part guarding to prevent employees from having any part of their body enter into the danger zone during the operating process. The machine was able to be operated with rotating parts exposed; on or about 7/25/17. c) Worksite: The employer did not ensure that Bonko Controls - Machine Technologies & Control, Inc. Straightener #3, Asset #2593, Equipment #ST03, Serial #501 187 was equipped with adequate rotating part guarding to prevent employees from having any part of their body enter into the danger zone during the operating process. The machine was able to be operated with rotating parts exposed; on or about 7/25/17. d) Worksite: The employer did not ensure that Bonko Controls - Machine Technologies & Control, Inc. Straightener #4, Asset #2594, Equipment #ST04, Serial #501 188 was equipped with adequate rotating part guarding to prevent employees from having any part of their body enter into the danger zone during the operating process. The machine was able to be operated with rotating parts exposed; on or about 7/25/17. e) Worksite: The employer did not ensure that Bonko Controls - Machine Technologies & Control, Inc. Straightener #5, Asset #2595, Equipment #ST05, Serial #501 189 was equipped with adequate rotating part guarding to prevent employees from having any part of their body enter into the danger zone during the operating process. The machine was able to be operated with rotating parts exposed; on or about 7/25/17. Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
