Straightener #3, Asset #2593, Equipment #ST03, Serial #501 187 was equipped with adequate rotating part guarding to prevent employees from having any part of their body enter into the danger zone during the operating process. The machine was able to be operated with rotating parts exposed; on or about 7/24/17.    b) Worksite: The employer did not ensure that Bonko Controls - Machine Technologies & Control, Inc. Straightener #2, Asset #2591, Equipment #ST01, Serial #501 185 was equipped with adequate rotating part guarding to prevent employees from having any part of their body enter into the danger zone during the operating process. The machine was able to be operated with rotating parts exposed; on or about 7/25/17.    c) Worksite: The employer did not ensure that Bonko Controls - Machine Technologies & Control, Inc. Straightener #3, Asset #2593, Equipment #ST03, Serial #501 187 was equipped with adequate rotating part guarding to prevent employees from having any part of their body enter into the danger zone during the operating process. The machine was able to be operated with rotating parts exposed; on or about 7/25/17.    d) Worksite: The employer did not ensure that Bonko Controls - Machine Technologies & Control, Inc. Straightener #4, Asset #2594, Equipment #ST04, Serial #501 188 was equipped with adequate rotating part guarding to prevent employees from having any part of their body enter into the danger zone during the operating process. The machine was able to be operated with rotating parts exposed; on or about 7/25/17.    e) Worksite: The employer did not ensure that Bonko Controls - Machine Technologies & Control, Inc. Straightener #5, Asset #2595, Equipment #ST05, Serial #501 189 was equipped with adequate rotating part guarding to prevent employees from having any part of their body enter into the danger zone during the operating process. The machine was able to be operated with rotating parts exposed; on or about 7/25/17.     Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
