29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  On or about; 07/17/17  Location: Ridge Hill Mall Compactors #1) Building H (Wet trash); #2) Building B (Cardboard); #4) Building A3 (Wet trash); #5) Building C (Cardboard); #6) Building A1 (Recyclables); #7) Building A1 (Cardboard); #8) Building A1 (Wet trash); #9) Building G (Cardboard); #10) Building D (Wet trash); and #11) Building E1 (Cardboard)   a) The employer failed to protect each employee by developing, documenting and utilizing procedures for the control of potential hazardous energy. Employees performing equipment maintenance were exposed to caught in between hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks;  On or about; 07/17/17  Location: Ridge Hill Mall Compactors #1) Building H (Wet trash); #2) Building B (Cardboard); #4) Building A3 (Wet trash); #5) Building C (Cardboard); #6) Building A1 (Recyclables); #7) Building A1 (Cardboard); #8) Building A1 (Wet trash); #9) Building G (Cardboard); #10) Building D (Wet trash); and #11) Building E1 (Cardboard)   a) Employees were exposed to caught in between and flying chips hazards while engaged in activities  in close proximity to or inside the unguarded compactors by not having any safety interlocks to prevent operation while unguarded or by having the safety interlocks overridden, while these compactors can run the complete cycle without holding/pushing the operating button.
