29 CFR 1926.451(b)(6): On scaffolds where scaffold planks were abutted to create a long platform, each abutted end did not rest on a separate support surface:      a) On or about July 27, 2017 and at times prior, Quest Masonry, Inc., as the creating and correcting employer, did not ensure that each abutted end of the two outer planks rested on a supported surface. Each abutted end of the outer planks was rested on the extended side of the outrigger planks. A foreman of Quest Masonry Inc. and Funez Stucco LLC were working on each end of the outer planks when one of the workers moved towards the other worker. The planks tilted to one side which caused both workers to fall approximately 40 feet to the ground.

Outrigger planks were not nailed, bolted, or secured to the outriggers; each abutted end of the outer planks was not rested on a supported surface; and guardrail systems were not installed along all open sides of the platform. A foreman of Quest Masonry Inc. and Funez Stucco LLC were working on each end of the outer planks when one of the workers moved towards the other worker. The planks tilted to one side which caused both workers to fall approximately 40 feet to the ground.

The workers were exposed to approximately 40 feet fall hazards while adding outer planks at the fifth level of the scaffold.
