29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system.  At the jobsite located at 1047 William and Mary Court, Barnhart, Missouri, an employee(s) for Alejandro Ramon Santos were  observed installing underlayment on a residential roof without the use of fall protection.  The fall distance was approximately 10 feet to the ground below.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.   At the jobsite located at 1047 William and Mary Court, Barnhart, Missouri, an employee(s) for Alejandro Ramon Santos were observed using a portable ladders to access the roof that had not been extended at least 3 feet beyond the upper landing surface or edge.
