29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a)  2413 OXFORD AVENUE, HARRISBURG, PA - Employee(s) worked to gut and remodel a residential house, and the employer had not required employees to wear any type of head protection.  The home completely collapsed while employee(s) were working inside the structure which resulted with one employee trapped underneath the debris where the employee sustained minor injuries and was transported to the hospital, on or about July 12, 2017.  No abatement certification or documentation required.

29 CFR 1926.850(a): An engineering survey was not performed by a competent person to determine the conditions of the framing floors and walls and the possibility of unplanned collapse of any portion of the structure prior to permitting employees to start demolition operations:  (a)  2413 OXFORD AVENUE, HARRISBURG, PA - Employee(s) worked to remodel a residential house, and an engineering survey had not been performed prior to the work being started.  The home completely collapsed while employees were working inside the structure which resulted with one employee trapped underneath the debris where the employee sustained minor injuries and was transported to the hospital, on or about July 12, 2017.   No abatement certification or documentation required.
