29 CFR 1926.453(b)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift. (a) On or about July 26, 2017 and at times prior, four employees were exposed to a catapult hazard and subsequent fall of approximately twelve feet to the ground below while using the S-40 Genie aerial lift and were not belted to the basket or boom.

(a) On or about July 26, 2017 and at times prior, four employees were exposed to a fall of approximately twelve feet to the ground below while using the S-40 Genie aerial lift outside the manufactures recommended specifications.
