29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, shall be rigged in such a way that an employee can neither free fall more than 6 feet (1.8 m), nor contact any lower level. On or about July 13, 2017, an employee installing communications equipment was exposed to fall and struck-by hazards when the employer did not ensure the personal fall arrest system was rigged in such a way that the employee could not contact any lower level. Abatement documentation is required of this item in accordance with requirements of 29 CFR 1903.19(d).

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. On or about July 13, 2017, employees performing communication equipment installation were exposed to fall and struck-by hazards while working on the upper level when the employer failed to ensure they were adequately trained to recognize and minimize the hazards associated with fall. Abatement certification is required of this item in accordance with requirements of 29 CFR 1903.19(c).
