29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) In the fabrication area - Procedures were not developed and documented for controlling hazardous energy sources on machines and equipment, including the Park Industries Titan 2800 CNC Router. During the period between November 1, 2017 and December, 4, 2017, an employee provided service to the Titan CNC Router, including removing guards/covers over the tracks and spindle to apply grease to these parts of the machine. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices: (a) Employees performing service activities on machines and equipment, including greasing and lubricating the Titan 2800 CNC Router, were not trained to acquired the knowledge and skills required for the safe application, usage and removal of energy control devices. During the period of November 1, 2017 to December 4, 2017, an employee provided service to the Titan 2800 by removing the guards/covers over the tracks and spindle to apply grease to these parts of the machine without locking out the equipment. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
