29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. On or about July 19, 2017, at Poland Jr. High School, Alexandria, LA, the employer failed to ensure that employees wore appropriate eye protection while using a DeWalt Miter saw.

29 CFR 1926.405(g)(1)(iii): Flexible cords and cables were run through doorways, windows, or similar openings: On or about July 19, 2017 at Poland Jr. High School, the employer allowed employees to run flexible cords through doorways. An employee used the extension cord as a power source for a hardy box which was used to plug in a DeWalt Miter saw.
