29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) did not cover the following elements and actions and shall be done in the following sequence:                On or about July 18, 2017, employees were performing minor servicing and maintenance in the bagger area on the Tanabe machines without locking the equipment out, thereby employees were exposed to amputation hazards.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.            On or about July 11, 2017, employees were operating the Tanabe machine and the machine was not adequately guarded in the following areas, thereby exposing employees to amputation hazards.    a) The opening in the guard in the compression area of the Tanabe machine was greater than 2 inches wide.    b) The in-rolling rollers in the compression area of the Tanabe machine were not guarded.
