(a) On or about July 26, 2017 on the 6th floor unit of the Margaret Tietz Nursing and Rehabilitation Center located at 164-11 Chapin Parkway, Jamaica, NY 11432    Employees were not able to open 3 exit route doors on the 6th floor unit from the inside at all times, following a fire drill exercise without keys, tools or special knowledge. The codes to release the doors were not provided to all employees during the time of the drill.    (b) On or about July 29, 2017 on the 6th floor unit of the Margaret Tietz Nursing and Rehabilitation Center located at 164-11 Chapin Parkway, Jamaica, NY 11432    Employees were not able to open 3 exit route doors on the 6th floor unit from the inside at all times, during a smoke condition which occurred on the roof directly above, without keys, tools or special knowledge. The codes to release the doors were not provided to all employees during the time of the smoke condition.        WRITTEN ABATEMENT CERTIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19.

(a) On or about July 26, 2017 on the 6th floor unit at the Margaret Tietz Nursing and Rehabilitation Center located at 164-11 Chapin Parkway, Jamaica, NY 11432    Employees were unable to exit the 6th floor unit through three emergency exit fire doors after the doors did not release automatically following the alarm system activation during a fire drill exercise.      (b) On or about July 29, 2017 on the 6th floor unit at the Margaret Tietz Nursing and Rehabilitation Center located at 164-11 Chapin Parkway, Jamaica, NY 11432    Employees were unable to exit the 6th floor unit through the C- stairwell emergency fire door after the doors did not release automatically following the alarm system activation during a smoke condition occurring on the roof above the 6th floor unit. Emergency personnel responding to the incident forced open the door in order to exit the unit.      WRITTEN ABATEMENT CERTIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19.
