29 CFR 1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: In the back of the warehouse area: on or about July 28, 2017 and at times prior thereto, employees were exposed to a fall hazard, while operating a BT Prime-Mover model #0E35 which had been modified without the approval of the manufacture to add an anchor point for the employees to attach their fall protection system.
