29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:      a)  Saw Mill; On or about August 8, 2017 and at times prior thereto the employer exposed his employees to potential hazardous energies in that employees were allowed to work on the re-saw which has multiple energy sources such as electric and hydraulic without those employees having a written machine specific lock out procedure to ensure that those energy sources were controlled while the employees were performing maintenance or servicing on the machine.

29 CFR 1910.219(c)(2)(i): All exposed part(s) of horizontal shafting seven (7) feet or less from floor or working platform were not protected by stationary casing(s) enclosing shafting completely or by trough(s) enclosing sides and top or sides and bottom of shafting:  a)  Sam Mill Re-saw Area; On or about August 8, 2017; the employer exposed employees to amputation hazards in that employees were allowed to work in close proximity to an unguarded rotating shaft on the Re-saw infeed conveyor.

29 CFR 1910.265(c)(4)(iv): Elevated platform(s), 4 feet or more from the floor level, were not provided with a standard railing:   a)  Saw Mill; On or about August 8, 2017: the employer exposed employees to an 8 foot fall hazard in that employees changing the band saw blade on the re-saw were allowed to walk and work next to the edge of an elevated platform while no standard railing or other method of fall protection was in place.
