29 CFR 1910.219(f)(3): Sprockets and chains. All sprocket wheels and chains shall be enclosed unless they are more than seven (7) feet above the floor or platform. Where the drive extends over other machine or working areas, protection against falling shall be provided. This subparagraph does not apply to manually operated sprockets. On July 27, 2017, in the Production Area, employees were exposed to a caught in-between, nip point hazard from an exposed sprocket on a direct drive plastic belt conveyor located approximately 18 inches off the ground which was not enclosed.

29 CFR 1910.36(g)(2): An exit access must be at least 28 inches (71.1 cm) wide at all points. Where there is only one exit access leading to an exit or exit discharge, the width of the exit and exit discharge must be at least equal to the width of the exit access. In the Production Area, the exit access from each bandsaw operator's work area leading out to an exit measured 12 inches.
