OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: a) 1738 Forrest Avenue, Dover Christian Church, Dover, DE: On or about August 1, 2017, employees engaged in roof truss installation were exposed to injury from being struck by collapsing truss components as the trusses were not stabilized with temporary bracing, including ground bracing and top chord temporary lateral restraints (TCTLR).

29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) Dover Christian Church, 1738 Forrest Avenue, Dover, DE - On or about July 31, 2017, the employer failed to ensure that subcontractor employees used fall protection while working at a height of approximately 11 feet above grade. On July 31, 2017, a Menno's Construction employee fell to his death while setting trusses.
