29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification: a) Defense Logistics Agency, First Floor, North Tower: On or about 07/26/2017, two 50-foot yellow extension cords were not used in accordance with its listing and labeling, the extensions were plugged into each other to supply power to items in and on top of a GreenLee gang box thereby exposing employees to possible fire. b) Defense Logistics Agency, First Floor, North Tower: On or about 07/26/2017, the mini refrigerator was not used in accordance with its listing and labeling in that the refrigerator was not plugged directly into a wall receptacle.
