29 CFR 1926.453(b)(2)(iv): Employee(s) were not standing firmly on the floor of the basket, were sitting or climbing on the edge of the basket, or were using planks, ladders, or other devices for a work position:  a. On or about July 27, 2017, three employees were climbing onto the external edge of the basket of a JLG aerial lift located at the side of the building, an elevation of 26 feet or more above the ground, exposing themselves to a 26 feet fall hazard. Employees were not protected by a fall protection system.

On or about July 27, 2017, three employees working from a JLG aerial lift were not wearing a body belt and/or a lanyard, exposing themselves to a 26 feet fall hazard.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a. On or about July 27, 2017, three employees performing wrapping on the sidewall of the building were exposed to a 26 feet fall hazard, because they were not protected by a fall protection system. There were no guardrails, safety nets, or personal fall protection equipment installed.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates, mud sills or other adequate firm foundation:  a. On or about July 27, 2017, on the scaffold at the side of the building, the support posts did not have a firm foundation in that the 4-inch by 4-inch framing grade lumber post rested directly on disturbed dirt without base plates, mudsills, or other foundation, exposing employees to an 11.6 feet fall hazard.
