29 CFR 1926.453(b)(2)(iv): Employees were not always standing firmly on the floor of the basket of extensible and articulating boom platforms.   a) 1529 Rand Road- On July 31, 2017, employees were exposed to the hazard of falling up to approximately 15 feet when climbing to and from the basket of a JLG 600S Boom Lift aerial lift platform while the lift was used as a means of egress to and from the roof of the building.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.453(b)(2)(v): A full body harness was not worn and a lanyard attached to the boom or basket when working from an aerial lift.  a) 1529 Rand Road- On July 31, 2017, employees were exposed to the hazard of falling approximately 15 feet from a JLG 600S Boom Lift aerial lift platform because a full body harness was not worn with a lanyard attached to the boom or basket while working from the aerial lift. 29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  a) On or about July 31, 2017, the employer did not instruct each employee in the recognition and avoidance of unsafe conditions while working from an aerial lift.  Employees were being transported in the aerial lift to perform welding and to gain access to a roof.
