29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:    Tank erection area:  On or about July 26, 2017, employees were erecting a steel tank section by section with the use of a crane.  Some employer were not wearing the available hardhats.  This exposed them struck by hazards.

29 CFR 1926.350(a)(10): Oxygen cylinders in storage were not separated from fuel-gas cylinder or combustible materials (especially oil or grease), a minimum distance of 20 feet or by a noncombustible barrier at least 5 feet high having a fire-resistance rating of at least one-half hour:    Tank erection area:  On or about July 26, 2017, and times prior to, employees engaged in the erection of steel tanks used  and worked around acetylene and oxygen cylinders.  Spare oxygen cylinders were stored with the acetylene in use.  This exposed the employees to the hazards of fire.

29 CFR 1926.352(d): Suitable fire extinguishing equipment was not immediately available in the work area(s) where welding, cutting, or heating was being performed:    Tank erection area:  On or about July 26, 2017, and times prior to, employees were engaged in the erection of two new storage tanks at a liquid feed mill using acetylene/oxygen to cut the steel tank components and arc welding to assemble the components.  There were no fire extinguishers posted for immediate use exposing the employees to the hazards of fire.

29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites:  Tank erection area:  On or about July 26, 2017, and times prior to, employees erecting a steel tank use an angle grinder powered through an extension cord.  The cord was connected to a receptacle in the adjoining building.  No ground fault protection was provided exposing the employees to the hazard of electrical shock.
