Instance 1:   One blade brake of the Idaco four blade saw was inoperative exposing the employee catching boards exiting the machine.  The brake was designed to immediately stop the blade if the emergency stop cable was bumped.    Instance 2: Both Apollo Mango saws did not have blade guarding for when saw was in the home position and operators did not adjust the barrier guard that was designed to prevent them from getting their hands in the path of the blade.
