29 CFR 1926.501(b)(13):     "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system.    On 7/14/2017, employees from Parker Seeley Design and Construction LLC were working on the roof of a residential building located at 927 South Fourth Street, Saint Charles, MO.  Three employees were engaged in roofing work approximately 11 feet above a lower level. Exposed employees were not utilizing guardrail systems, a safety net system, or a personal fall arrest system.  The employees were observed on the roof of the building without fall protection, exposing themselves to a fall to the ground below.

29 CFR 1926.451(g)(1)  Each employee on a scaffold more than 10 feet (3.1 m) above a lower level was not protected from falling to that lower level.  On 7/14/2017, an employee Chris Walker was installing an ice/ rain shield on the side of a residential building located at 927 South Fourth Street, Saint Charles, MO. The employee was on performing this work on a ladder jack scaffold approximately 10-11 above the ground. The employee was not protected from falling to the lower level.

29 CFR 1926.451(e)(1):    When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface were not used. Support ladders were used as a means of access.    On 7/14/2017, An Employee from Parker Seeley Design and Construction LLC was performing roofing operations from a ladder jack scaffold on a residential building located at 927 South Fourth Street, Saint Charles, MO. Sufficient access to the scaffold platform was not available.

On 7/14/2017, An Employee from Parker Seeley Design and Construction LLC was performing roofing operations on a residential building located at 927 South Fourth Street, Saint Charles, MO. The employee was working from ladder jack scaffolding on the front of a detached garage structure approximately ten to eleven feet above the ground.  The ladder jack scaffolding consisted of three ladders and two platforms.  The platforms were bridged together on the center ladder.

Eye protection was not used by an employee using pneumatic nail gun.
