Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system. On 7/14/2017, employees from Parker Seeley Design and Construction LLC were working on the roof of a residential building located at 927 South Fourth Street, Saint Charles, MO. Three employees were engaged in roofing work approximately 11 feet above a lower level. Exposed employees were not utilizing guardrail systems, a safety net system, or a personal fall arrest system. The employees were observed on the roof of the building without fall protection, exposing themselves to a fall to the ground below.

The employee was on performing this work on a ladder jack scaffold approximately 10-11 above the ground. The employee was not protected from falling to the lower level.
