29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: Church expansion site: On or about July 27, 2017, employees engaged in steel erection activities were not wearing hard hats. This exposed employees to struck by hazards.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems. Church expansion site: On July 27, 2017, employees were engaged in the steel construction of a church expansion. One employee was working on an existing metal roof without any fall protection. This exposed the employee to fall of over 15 feet.
