29 CFR 1926.404(b)(1)(ii): Where an assured equipment grounding program was not utilized, receptacles were not protected with ground-fault circuit-interrupters when on a two-wire, single-phase portable or vehicle-mounted generator rated more than 5kW, or where the circuit conductors of the generator were not insulated from the generator fame and all other grounded surfaces:    a)    Defense Logistics Agency, Building 34: On or about 07/27/2017, the Predator 6,500 peak watts generator used by employees to supply power to a chop saw, battery charger and table saw was not provided with ground fault interrupter protection, thereby exposed employees to a possible electrical shock hazard.       Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:  a)    Defense Logistics Agency, Building 34: On or about 07/27/2017, the UTILITECH 5-outlet adapter used by employees to supply power to a chop saw, battery charger, and table saw the path to ground was not continuous and permanent in that the ground prong was missing from the male attachment plug thereby exposed employees to a possible electrical shock hazard.   Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.502(b)(2): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structural members were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches (53 cm) high:    a)    Defense Logistics Agency, Building 34: On or about 07/27/2017, a middle rail was not installed on the prefabricated 5'x 5' frame scaffolding system erected approximately 37 feet high along the building exterior, as a result employees were exposed to a possible fall hazard.     Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.      a)    Defense Logistics Agency, Building 34: On or about 07/27/2017, an employee was on the roof without fall protection loading wood into a basket attached to the forks of a rough terrain forklift truck as a result employee was exposed to a 27 feet fall hazard.     Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
