29 CFR 1926.451(c)(1): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by guying, tying, bracing, or equivalent means, as set forth in subparagraphs (i) - (iii) of this paragraph.  Site: 354 A Prospect Ave, Brooklyn, NY.  On or about 8/1/2017  a)  The employer used ropes to anchor the scaffold frames to the window guards of the building for stability.  Employees worked from the 5th frame of a tubular welded frame scaffold.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  Site: 354 A Prospect Ave, Brooklyn, NY.  On or about 8/1/2017  a) Employees worked on a tubular welded frame scaffold and it did not have safe means of access.  Employees climbed the cross bars and frames to access the scaffold.

Site: 354 A Prospect Ave, Brooklyn, NY.  On or about 8/1/2017  a) The scaffold used on site was not erected under the supervision and direction of a competent person qualified in scaffold erection.  The scaffold deficiencies consisted of but not limited to working platforms not fully planked, missing guards rails and endrails, was not tied to prevent tipping, and had no safe means of access.

On or about 8/1/2017  a) An employee hoisting material from the scaffold platform to the ground level was exposed to a fall of approximately 24 feet.  The employee was not provided with fall protection.
