On or about 7/26/2017, Oscar L. Esquivel Rodriguez was installing shingles on a roof of residence located on lot #64 Honeysuckle Creek Drive in Wentzville, MO. The owner and an employee were not wearing eye protection while using pneumatic nail guns to install shingles.

On 7/26/2017, Oscar L. Esquivel Rodriguez was installing shingles on a roof of residence located on lot #64 Honeysuckle Creek Drive in Wentzville, MO. The owner and an employee were not utilizing guardrail systems, a safety net system, or personal fall arrest systems. They were observed on the roof of the building without fall protection, exposing them to a fall to the ground below.
