(a)  Global Underground Corp. at 8650 N Decatur St., Westminster, CO 80031: On and preceding 7/28/17, the creating and exposing employer did not ensure employees were not permitted underneath loads handled by lifting or digging equipment. Employees were located inside a trench box rigging out equipment used for sewer line replacement. The pull head was rigged out by a track hoe utilizing a single chain. Employees were in the trench box, underneath the load, when the pull head fell out of the chain and struck an employee. This condition exposed employees to struck by hazards.
