On 7/26/2017, All Seasons Exteriors, LLC was installing shingles on a roof of residence located on lot #580 Lost Canyon Blvd., in Wentzville, MO. An employee was not wearing eye protection while using a pneumatic nail gun to install shingles.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest systems. On 7/26/2017, All Seasons Exteriors, LLC was installing shingles on a roof of residence located on lot #580 Lost Canyon Blvd., in Wentzville, MO. The crew leader and four employees were not utilizing guardrail systems, a safety net system, or personal fall arrest systems. They were observed on the roof of the building without fall protection, exposing them to a fall to the ground below.
