Such activities shall be performed only by experienced and trained employees selected for such work by the competent person. On or about August 2, 2017, during the painting of a residence located at 1366 Beach Avenue, Lakewood, Ohio, the employer did not ensure a ladder jack type scaffold was erected under the supervision and direction of a competent person qualified in scaffold erection.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold shall be protected by a personal fall arrest system; On or about August 2, 2017, during the painting of a residence, located at 1366 Beach Avenue, Lakewood, Ohio, the employer did not ensure an employee working from a ladder jack type scaffold was protected from falling to a lower level by a personal fall arrest system. The employee was working approximately 21 feet above the ground.
