Location: Outdoor steel storage container    On or about 8/1/17, the employer stored eleven containers of fuel in an outdoor steel storage container without a gravity or mechanical ventilation system exposing employees to fire hazards.

Location: Yard area  On or about 8/1/17, an employee operating a Husqvarna, model 357 XP chainsaw was exposed to flying objects and eye injuries due to wearing prescription glasses without side-shields that did not offer the same protection as safety glasses.
