29 CFR 1910.303(b)(1)(iv): Employer did not maintain electrical insulation;  (a) On or about 8/7/2017 at Brooklyn Mitsubishi Service Center located at 5690 Avenue D Brooklyn, NY 11203, workers using a Haulotte Optimum 1930E, Electric Scissor Lift  were exposed to fire and electrical shock hazards from a damaged power cord with exposed insulation.  NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT CERTIFICATION OF ABATEMENT FOR THIS ITEM. FAILURE TO COMPLY MAY RESULT IN AN ADDITIONAL PENALTY.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  (a) On or about 8/7/2017 at Brooklyn Mitsubishi Service Center located at 5690 Avenue D Brooklyn, NY 11203, employees performing car repairs were exposed to chemicals such as but not limited to N-heptane, and were not provided with a written Hazard Communication Program.   NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000 AS PER 29 CFR 1903.19.
