29 CFR 1926.28(a): The employer did not require the wearing of appropriate personal protective equipment in all operations where there is an exposure to hazardous conditions:    a) West side of home, on or about 8-2-17:  One employee was on the ground below employees performing roofing work overhead, and was not wearing head protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):    a) East side of building, on or about 8-2-17:  Two employees were working on a sloped roof (6:12 pitch) and were not protected from falls, exposing the employees to a fall hazard of 17 feet 4 inches to the ground below.     b) East side of building, on or about 8-2-17:  Two employees were working on a sloped roof (6:12 pitch) and were not protected from falls, exposing the employees to a fall hazard of 16 feet to a deck below.     c) West side of building, on or about 8-2-17:  Two employees were working on a sloped roof (6:12 pitch) and were not protected from falls, exposing the employees to a fall hazard of 16 feet 3 inches to the driveway below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a) West side of building, north end, on or about 8-2-17:  Three employees were accessing a roof using a portable ladder that was 26 inches above the landing surface, exposing the employees to a fall hazard of  14 feet 4 inches.     b) West side of building, south end, on or about 8-2-17:  Three employees were accessing a roof using a portable ladder that was 28 inches above the landing surface, exposing the employees to a fall hazard of  11 feet 6 inches.
