29 CFR 1926.502(i)(3): All covers were not secured when installed so as to prevent accidental displacement by the wind, equipment, or employees: (a) 1401 Spruce St., Philadelphia, PA 19102, 16th Floor - The employer did not ensure a cover installed over a floor hole, at a height of approximately 12 feet above the lower level, was secured, thus exposing employees to fall from height hazards. This condition was observed on or about August 2, 2017.
